Physical Therapy Billing Information

Regular access and communication with your physical therapist is included for one month following each in-person session. This ensures continued guidance through your healing process.

A complimentary virtual consultation will be scheduled the week after your session to ensure your exercises are effective and properly performed.

Package Options (In-Home Sessions):

• 10 sessions – $2,000

• 6 sessions – $1,350

• 4 sessions – $960

Individual Services:

• Single in-home session – $250
  (Includes a virtual follow-up consultation)

• Virtual Physical Therapy Consultation only – $100

Billing Notes:

• Out-of-network services can be provided at Integrated Martial Arts and Fitness with insurance reimbursement.  
  (Insurance billing is not available for mobile/in-home treatments.)

• Payment is due at the time of service.

• A superbill will be provided for you to submit to your insurance carrier for potential reimbursement.
